3. Project description each Eagle used for his qualification
4. Picture of each Eagle project.
5. Date of Board of Review

If somehow I don't have the information for a certain Eagle and you can help me fill in the gaps, please follow the directions below and send it to me.  If you are sending in historical (rather than for a current project) no parental signature is obviously not necessary.

Historical help can be sent to Glen Forister.  Eagle applicants hand in this document with their notebook.
